Library Cards are issued to
To get a library card everyone must fill out an application form. Adults must show 2 forms of identification with their current address on it.
Valid forms of identification are:
A post office box is not acceptable proof of address.
High School students may use a Student I.D. card as identification.
Children under the age of 14 do not require proof of address, but must have the application form signed by a parent or guardian.
Property owners must bring in a current tax bill. Everyone residing at that address is entitled to a library card.
To apply for a business card, a letter from the employer is required.
All cards are renewed annually, and proof of address must be shown each year.

* You may borrow a total of 15 videocassettes.
**You may borrow a total combination of 10 cassette books.
PLEASE NOTE: During periods of heavy demand for class assignments there is a
limit on Children's non-fiction of two books per subject.
You may request a 3-week loan for new adult fiction over 450 pages. Extended loans on certain items (6 weeks, no renewals) are available to PML card holders on vacation.
Patchogue-Medford Library cardholders may reserve all library materials, except videos that are new or in high demand (starred). Items not available may be requested through interlibrary loan.
Books, records, cassettes, compact discs, CD-ROMs and pamphlets may be renewed twice in person, by telephone or online, unless there is a demand for the materials. Framed art, non-starred DVD's and VHS's may be renewed once. You may not renew 7-day or 14-Day loan items. AV materials with a star (new or in high demand), E-book readers, fishing rods or puzzles.
Patchogue-Medford library cardholders may borrow books, articles, reports, census reels and non-fiction videos that are not available in our library through interlibrary loan.
You are responsible for:
A notice is sent for 3 and 7 day loan when it is 7 days overdue. A notice is sent for other material when it is 3 weeks overdue. The second and final notice is a bill. All borrowing privileges will be withdrawn until the material is returned or paid for. A service charge may be added to delinquent accounts.
If a library card is lost or stolen, the library should be notified immediately.

non-district borrowers.

YOU MAY NOT BORROW:
You may use public typewriters, public computers, record players and
audio cassette players in the library.
You may register for programs, trips and career counseling unless they
are restricted to Patchogue-Medford residents.
Your home library may provide interlibrary loans, reserves and library
card renewals, except for starred items.
